How these plans work

CarePlus Health Plans, Inc. Medicare Advantage approved HMO plans let you choose your own primary care doctor from an extensive list of physicians. Together, you and your doctor coordinate all of your care. You even have the option to change your Primary Care Physician at any time. Your Primary Care Physician, called a PCP, is your partner in health and will send you to specialists or hospitals when you need specialized care.

CarePlus Health Plans, Inc. Medicare Advantage approved HMO plans cover all the benefits of Original Medicare and much more, including:

You must reside in the service area of the Plan. You must continue to pay your Medicare Part B premium, if not otherwise paid for under Medicaid or by another third party.
